Summer Camp Registration

1. In Person:

Download and complete Registration Form, and submit with payment at our program location in the following time. Form can also be obtained at our program location.

Your registration will be confirmed right away.

2. Online:

(1) Register the program online directly. Enter >.

(2) A confirmation email will be sent to your email address once the registration information is received.

(3) Sign the registration form and summit your payment either in person or by mail (see below).

3. Email:

(1) Download and complete Registration Form, and email to office@threeflavors.org.

4. Mail:

Download and complete Registration Form, then mail with your payment to our accounting address.
A confirmation email will be sent to your email address once the registration form and/or payment are received.

Submit Payment

After the confirmation is received, please print and sign the registration form, then submit with your payment either in person or mail to the following our accounting address within 5 days.
